Recommendation PREAMBLE:

From the investigation it was identified failure of HCAA during audits to detect noncompliance with the approved Operator’s OMM procedures, in terms of the risk assessment and analysis accomplishment before the operation on a new route.

RESPONSE to Safety Recommendation:

The second response of [Hellenic] Civil Aviation Authority (HCAA) to AAIASB Safety Recommendation 2022-08 was received by email on 21/3/2023.

[Second HCAA Response]:

“ HCAA's internal procedures have been reviewed and revised, and the inspection plan for the year 2022-2023 includes inspections in all areas of control of organizations in accordance with the European regulation 965/2012 with new EASA inspection Check-Lists that are available to inspectors of HCAA from 01/01/2023. The specific new HCAA procedure fully ensures compliance with the approved procedures for the inspection of the Organizations. ”

[Hellenic National Agency for the Investigation of Air and Rail Accidents & Transport Safety (former AAIASB) assessed this response as “Adequate” on 03/04/2023 and changed the SR 2022-08 status to “Closed”]



History of previous response:

The [Hellenic] Civil Aviation Authority’s (HCAA) first response to AAIASB Safety Recommendation 2022-08 was received by email on 31/1/2023.

[HCAA’s first response]:

“ We would like to inform you that following the issuance of the Final Report [AAIASB Final Report 04/2022] and the safety recommendations of the Investigation Board [AAIASB] to the HCAA regarding the accident that occurred at the Poros-Galata Sea Channel on 20/8/2019, the Flight Operations Department of the Directorate for Flight Standards issued the following recommendations addressed to all air carriers. (Safety Recommendation 2022-07, 08)

Flight Safety Recommendation 2022-08

The [Hellenic] Civil Aviation Authority recommends that the Company/ies [operator(s)] include in every flight containing a landing/takeoff in a field not approved by the Authority, a risk analysis assessment prior to the commencement of the route (Risk Assessment) which will be a necessary accompanying document.

The [Hellenic] Civil Aviation Authority advises helicopter operators to keep PART C [Operations Manual, Part C] up to date with all possible aerodromes and landing fields. Alternatively keep a Directory of Helipads up to date with detailed information and instructions for the approach, landing and take-off taking into account all the possible difficulties that the crew may encounter.

In addition, companies are recommended in cases where a route and Take-off or Landing is required in fields that have been identified as areas of great difficulty, to use only crews that have recent experience on the specific route and designated as experienced by the air carrier. (The recommendation is sent to the helicopter operators).”

[Hellenic National Agency for the Investigation of Air and Rail Accidents & Transport Safety (former AAIASB) with its document 349/14.03.2023 assessed initially within the 60-day deadline the addressee’s initial response as “Partially adequate” and changed the status of Recommendation 2022-08 to “Open — Partly adequate” stating that the corrective actions described in the addressee’s response, although correct, do not refer to the actions that the HCAA should take to ensure compliance with the approved inspection procedures to the Organizations, asking for these actions to be completed.]
